The following games are games, I have broken a rule on:
- Dark Souls: Remastered
- Yu-Gi-Oh! Legacy of the Duelist
- Fallout 4
Yu-Gi-Oh! Legacy of the Duelist
8 achievements were unlocked the same second on January 9th, 2017. For these, I have no explanation. I did not cheat them in, but I cannot prove it either. If these are to be re-locked, then so be it.
The following two achievements obtained 11:25:34, September 5, 2021, were SAM'ed:
- Casual Duelist
- Playing For Real
Dark Souls: Remastered
I initially completed Dark Souls: Remastered 100%. I then chose to use SAM to lock all achievements and then went through and obtained them again.
This time around I used a friend to drop me the weapons needed for the weapon achievements towards the end of my playthrough, which according to OK-7 is allowed.
Using SAM to re-lock my achievement was NOT okay, though.
In this case, I am unsure what to do. Should all the achievements be relocked once more? If so, then so be it.
Fallout 4
There is no great excuse for this one.
After a long break from achievement hunting, I eventually came back to it during the X-mas holidays.
I ended up having fun and thoroughly enjoyed the game whilst obtaining the different endings in the main campaign. I found out about the command console, wanted to try different things, like leveling up my character to 300, godmode, and... That one... After realizing that caqs unlocked a whopping 21 achievements, I went to read the rules (it's been a long time), and to great remorse, I stumbled upon NA-12:
- Do not use console commands which directly and instantly unlock achievements.
I apologize for having done this and simultaneously thank the volunteer for taking time out of their day to go through this appeal, my account, my games, and my achievements.

Edit: I have changed the messages on Yu-Gi-Oh and Fallout 4 to say that the achievements are to be re-locked instead of getting re-locked after submitting the appeal. I really should read before doing, as the very first line in the "How to relock your achievements"-guide says to NOT do any steps without instructions from an appeal.
